1.Before Your First-Coming to Our Hospital

As a general rule, Kyushu University Hospital accepts patients with referral letters from other medical institutes and appointments.
*For consultation at the Dental Department, it is preferable that you bring the referral letter prepared by your dentist (You can receive consultation even without a referral letter). Please refer to here

2.Flowchart of the Outpatient Clinic

About block reception for outpatient visits

The Outpatient Ward at Kyushu University Hospital is divided into East and West Blocks, with the reception desks (for block reception) located on each floor.
At the block reception, reception staff checks patient arrivals, verify their health insurance certificates, confirms completion of consultations, and so on.

The basic patient consultation flow starts at the first visit reception desk or the registration machine on the ground floor of the Outpatient Ward, and then proceeds to consultation at the concerned clinical department via the block reception desk.
 

Flow of the first visit

Please bring necessary documents such as the consultation reservation paper, consultation information submission document and CD/film, along with your health insurance card.
Kyushu University Hospital may request that a patient provide his/her name (first name/surname) and birthday to prevent mix-ups among patients.

1)General Information

Receive the first visit reception number, “number tag”

2)First Visit Reception, Desk1


Check-in Sheet (left) , and Patient ID card
Submit necessary documents such as your appointment sheet, patient referral document (medical record) and CD/film images, along with your health insurance card.
Your Check-in sheet and Patient ID Card are issued. Check these documents.

3)CD Administration Room , next to Admission , Discharge & Perioperative Support Center , 1F

If you are bringing a CD with you, data on the CD will be imported.Submit your Check-in Slip and CD images. 
Once the data has been imported, receive back your Check-in Slip and CD.

 

4)Block Reception

Submit your Check-in Slip and consultation information submission document to the block reception desk printed on your Check-in sheet.
Depending on the clinical department that you are visiting for consultation, you may need to fill out the interview sheet for outpatients.
When the Check-in Number printed on the upper right corner of your Check-in sheet is displayed on the consultation announcement display monitor, enter the waiting or consultation room.

5)Consultation Room

Receive the doctor's consultation.You may need to undergo checkup(s) if necessary.
And , make a return-visit appointment.


 

6)Block Reception


Check-in sheet
Once consultation has been completed, submit your Check-in sheet. 
If you have an appointment for your next visit, your next appointment sheet will be issued. Check this paper.
If you need to pay for your current visit, the Check-in sheet stub will be issued. Check this stub.

7)Accounting , Desk4

Submit your “check-in sheet”, if you have an examination after consultation.

8)Cashier , Desk5 or Automatic Cashier

If you see your daily number written on the top right of your check-in sheet on the monitor, please go to a waiting room or enter a consultation room.

If you need to pay for your current visit, wait in front of the Cashier.
When Check-in Number is displayed on the payment display monitor, make your payment at either the Cashier or the payment machine using your Check-in sheet stub.
 

9)Outpatient Counter

As a general rule, Kyushu University Hospital fills out-of-hospital prescriptions (by which patients submit prescription filled by Kyushu University Hospital to pharmacies out of the hospital and receive medication at these pharmacies).
Submit your off-site prescription. Kyushu University Hospital sends a fax of your off-site prescription to your choice of pharmacy in advance.
